No fasting is required for hormone tests. We recommend a morning appointment (before 10am) for the most accurate hormone readings, as FSH and cortisol levels are most stable in the early morning. Stay well hydrated — it makes the blood draw easier and quicker.

Understanding Your Results
FSH and LH tell a story together. During perimenopause, FSH rises as your pituitary gland works harder to stimulate the ovaries. A rising FSH with falling oestradiol suggests the ovaries are responding less — a hallmark of the perimenopausal transition. But levels fluctuate month to month, which is why a single "normal" result does not rule perimenopause out.
Thyroid should always be checked alongside hormones. Hypothyroidism (underactive thyroid) causes fatigue, weight gain, brain fog, depression, dry skin and joint pain — symptoms identical to perimenopause. It affects women 5-8 times more often than men and commonly emerges in your 40s. Testing both together means you do not spend years treating the wrong condition.
One blood test is not always enough. Perimenopausal hormone levels can swing dramatically — your FSH might be elevated one month and normal the next. If your symptoms are strong but your first results look borderline, retesting 4-6 weeks later can provide the pattern your GP needs to prescribe with confidence. Can a blood test confirm perimenopause?
Blood tests can strongly support a perimenopause diagnosis, but no single test gives a definitive yes or no. FSH and oestradiol levels fluctuate significantly during perimenopause — a raised FSH with low oestradiol is suggestive, but levels can be normal one month and abnormal the next. This is why we recommend testing alongside a symptom review, and sometimes repeat testing 4-6 weeks later. The real value of blood tests is ruling out other causes (thyroid disease, iron deficiency, vitamin D deficiency) that mimic perimenopause and need different treatment.
What FSH level indicates menopause?
An FSH level consistently above 30 IU/L, combined with no period for 12 months, is generally considered to confirm menopause. During perimenopause, FSH levels often fluctuate between 10-50 IU/L as the ovaries still produce oestrogen intermittently. A single "normal" FSH does not rule out perimenopause — context matters as much as numbers, which is why we provide clear interpretation with every result.
Why should I test my thyroid at the same time?
Thyroid dysfunction and perimenopause share almost identical symptoms: fatigue, weight gain, brain fog, mood changes, irregular periods, hair thinning and joint pain. Up to 1 in 8 women develop a thyroid condition in their lifetime, and it often emerges in the 40s — exactly when perimenopause begins. Should I stop HRT before having a blood test?
It depends on the purpose of the test. If you want to check your baseline hormone levels (for example, to see if you still need HRT), you would typically stop for 4-6 weeks before testing — but only with your prescribing doctor's guidance. If you are testing to see whether your current HRT dose is adequate, test while on treatment. We can advise when you book, and our results report will account for HRT use.
How often should I retest my hormones?
For most women, retesting every 6-12 months is helpful during the perimenopausal transition — especially if symptoms change, HRT is adjusted, or the initial results were borderline. Thyroid function should be rechecked 6-8 weeks after starting medication and then annually. Vitamin levels can be rechecked 3 months after starting supplementation to ensure levels have improved.
Can I get HRT from these results?
We provide diagnostic blood tests and detailed interpretation, but we do not prescribe HRT directly. Your results report is designed to give your GP or menopause specialist the evidence they need to prescribe confidently. Many GPs are happy to initiate HRT with clear blood results supporting the decision. If your GP is not supportive, we can recommend private menopause clinics who accept our reports.
